Welcome to Jameson Camp’s printer friendly registration for Discover Your Impact Summer Camp 2010!  This summer our campers will learn more about what they are capable of and how they can impact the world around them.

INSTRUCTIONS:

Please see the lists below for documents necessary for camper registration.  Documents are divided into two categories:  Required Forms and Optional Forms.  Please print and complete ALL REQUIRED documents.   

Someone from Jameson Camp will follow up with you after your camper is confirmed in his or her session and a payment assessment has been performed.  Please note that you are required to submit a signed copy of the Camper Health Form. 

PAYMENT NOTICE!

Registration for Summer Camp 2010 requires a $25 deposit for each application due immediately.  Jameson Camp accepts cash, money order, or credit cards (Visa and Mastercard Only.  NO PERSONAL CHECKS.

Health Form (due by first day of camp session)
Program Participation Agreement (due now)
Challenge Course Participation Agreement (due now)

OPTIONAL FORMS:

Payment Contract (Mandatory if an outside party is sponsoring your camper)
Recommendation Form (formerly Referral Form)
